UP Plans Free Emergency Treatment for Accident Victims for Up to 48 Hours
Proposed UPTEN network will connect ambulances, hospitals and trauma centres, with 1,200 ALS ambulances and a target of saving up to 12,000 lives annually
Lucknow: People facing serious accidents or critical medical emergencies in Uttar Pradesh may soon be able to receive emergency treatment without worrying about immediate payment, as the state government works on a proposal to provide free care for up to 48 hours.
The initiative is intended to ensure that financial uncertainty does not become a reason for delaying urgent medical intervention.
The proposed system is being developed around the principle of the “golden hour”, the crucial first 60 minutes after a traumatic injury or the onset of a life-threatening medical condition.
Under the plan, ambulances, hospitals, trauma facilities, doctors, paramedics and emergency command centres would operate as parts of a single coordinated network.
The proposed Uttar Pradesh Trauma and Emergency Network (UPTEN) is designed to make the first hour of emergency care more effective by ensuring that patients are assessed and directed towards appropriate treatment without unnecessary delays.
The state has set an ambitious objective of preventing an estimated 10,000 to 12,000 deaths every year through faster and better-coordinated emergency response.
Officials believe the proposed network could bring a major shift in the state’s emergency-care model.
Instead of treating ambulance travel merely as transportation to a hospital, the system would aim to initiate appropriate medical assessment and treatment as early as possible, including while the patient is being transported.
UPTEN is envisaged as a common framework for emergency services that currently function through separate systems.
Trauma, burn, cardiac, stroke and other critical-care facilities would be brought onto the integrated platform.
The proposed network would comprise 173 trauma and emergency medicine units, including 11 Level-1 apex centres, 36 Level-2 regional centres and 126 Level-3 district units.
Additional Chief Secretary (Health) Amit Ghosh said the initiative is focused on eliminating avoidable gaps between the time an emergency call is received, an ambulance reaches the patient, and treatment begins at the appropriate medical facility.
A major operational component of the plan is the proposed integration of 1,200 Advanced Life Support (ALS) ambulances.
Equipped to provide critical care on the move, these ambulances would allow medical teams to begin evaluating and stabilising seriously ill or injured patients before arrival at a hospital.
The state is also planning 18 command centres at the divisional level. These centres are expected to serve as coordination points for emergency calls, ambulance allocation, information on available hospital beds and access to specialist medical services.
Under the proposed framework, emergency response times are targeted to fall by 30 to 50 per cent.
The reduction could potentially provide nearly 10 additional minutes for doctors and emergency teams to make crucial medical decisions before the patient reaches the hospital.
Technologies such as real-time data sharing, digital alerts and artificial-intelligence-based triage are also being considered as part of the system.
For families caught in a medical crisis, the larger objective is to remove uncertainty from one of the most critical stages of emergency care: determining where a seriously injured or critically ill patient should be taken and ensuring that the appropriate treatment is ready when the patient arrives.
The guiding approach, officials said, will be “the right patient, right department, right time.”
The proposal also envisages a three-level first-responder mechanism that would bring police personnel, ambulance crews, fire services, highway patrol units, Home Guards and trained members of the public into the emergency-response chain.
The idea is to enable basic assistance to begin at the scene while specialised medical support is still on its way.
To support the long-term functioning of the network, the government is examining a dedicated financial and staffing framework for UPTEN.
Among the proposals under consideration are the creation of an Emergency Medicine Officer (EMO) cadre, AIIMS-like salary structures and additional incentives aimed at attracting specialist doctors to emergency services.
The emphasis on the golden hour reflects a widely accepted principle in emergency medicine: the sooner a patient with severe trauma, heart attack, stroke, or another life-threatening condition receives appropriate clinical care, the better the chances of survival and recovery.
Delays can increase the risk of complications, making rapid assessment and treatment particularly important during the earliest stage of an emergency.

लखनऊ: उत्तर प्रदेश में सड़क हादसे या किसी गंभीर चिकित्सकीय आपात स्थिति में फंसे मरीजों को शुरुआती उपचार के दौरान पैसों की चिंता से राहत देने की तैयारी है।
राज्य सरकार ऐसी व्यवस्था पर काम कर रही है, जिसके तहत दुर्घटना या गंभीर मेडिकल इमरजेंसी के बाद मरीज को 48 घंटे तक मुफ्त आपात चिकित्सा उपलब्ध कराने का प्रस्ताव है।
इसका उद्देश्य यह सुनिश्चित करना है कि इलाज शुरू करने में आर्थिक अड़चन जानलेवा देरी का कारण न बने।
इस पूरी योजना का केंद्र ‘गोल्डन ऑवर’ यानी आपात स्थिति के शुरुआती 60 मिनट हैं। चिकित्सा जगत में इस अवधि को बेहद निर्णायक माना जाता है, क्योंकि इसी दौरान त्वरित जांच और उचित उपचार मिलने से मरीज की जान बचाने तथा गंभीर जटिलताओं को रोकने की संभावना बढ़ जाती है।
प्रस्तावित व्यवस्था में एंबुलेंस से लेकर अस्पताल, ट्रॉमा सेंटर, डॉक्टर, पैरामेडिकल स्टाफ और कमांड सेंटर को एक साझा तंत्र में जोड़ा जाएगा।
राज्य में प्रस्तावित उत्तर प्रदेश ट्रॉमा एंड इमरजेंसी नेटवर्क (UPTEN) का मकसद मरीज को महज अस्पताल तक पहुंचाना नहीं, बल्कि रास्ते से ही उसके लिए उचित चिकित्सा व्यवस्था सक्रिय करना है।
सरकार ने इस नेटवर्क के माध्यम से हर वर्ष करीब 10,000 से 12,000 लोगों की जान बचाने का लक्ष्य रखा है।
अधिकारियों के मुताबिक, UPTEN लागू होने के बाद प्रदेश की आपात चिकित्सा व्यवस्था के काम करने के तरीके में बड़ा बदलाव आ सकता है।
मौजूदा सोच में जहां एंबुलेंस का मुख्य काम मरीज को अस्पताल पहुंचाना माना जाता है, वहीं नई व्यवस्था में सफर के दौरान ही मरीज की स्थिति का आकलन, प्राथमिक उपचार और जरूरी चिकित्सकीय हस्तक्षेप शुरू करने पर जोर होगा।
योजना के तहत अभी अलग-अलग तरीके से संचालित ट्रॉमा, बर्न, हृदयाघात, स्ट्रोक और अन्य गंभीर आपात सेवाओं को एक साझा प्लेटफॉर्म पर लाने की परिकल्पना की गई है।
इसके लिए प्रदेश में 173 एकीकृत ट्रॉमा एवं इमरजेंसी मेडिसिन यूनिट प्रस्तावित हैं। इनमें 11 लेवल-1 एपेक्स हब, 36 लेवल-2 क्षेत्रीय केंद्र और 126 लेवल-3 जिला स्तरीय इकाइयां शामिल होंगी।
अपर मुख्य सचिव (स्वास्थ्य) अमित घोष के अनुसार, योजना का प्रमुख उद्देश्य आपात कॉल से लेकर एंबुलेंस की तैनाती और मरीज के लिए उपयुक्त अस्पताल में इलाज शुरू होने तक होने वाली देरी को कम करना है।
बेहतर तालमेल के जरिये गंभीर मरीजों को कम समय में सही चिकित्सा सुविधा उपलब्ध कराने की कोशिश की जाएगी।
प्रस्तावित नेटवर्क की रीढ़ 1,200 एडवांस्ड लाइफ सपोर्ट (ALS) एंबुलेंस होंगी। इन्हें केवल परिवहन का साधन न रखकर चलती-फिरती आपात चिकित्सा इकाई के रूप में इस्तेमाल करने की योजना है।
इससे अस्पताल पहुंचने से पहले ही गंभीर मरीज की जांच और जरूरी स्थिरीकरण शुरू किया जा सकेगा।
आपात सेवाओं के संचालन और निगरानी के लिए प्रदेश में 18 मंडलीय कमांड सेंटर स्थापित करने का प्रस्ताव भी है।
इन केंद्रों से इमरजेंसी कॉल, एंबुलेंस भेजने, अस्पतालों में उपलब्ध बेड और विशेषज्ञ डॉक्टरों की सेवाओं से जुड़ी जानकारी का समन्वय किया जाएगा।
सरकार आपात प्रतिक्रिया समय में 30 से 50 प्रतिशत तक कमी लाने का लक्ष्य लेकर चल रही है। अधिकारियों का मानना है कि प्रतिक्रिया समय घटने से मरीज के अस्पताल पहुंचने से पहले महत्वपूर्ण चिकित्सकीय फैसलों के लिए करीब 10 अतिरिक्त मिनट मिल सकते हैं।
व्यवस्था को मजबूत करने के लिए रियल-टाइम नेटवर्किंग, डिजिटल अलर्ट और एआई आधारित ट्रायेज जैसी तकनीकों के इस्तेमाल पर भी विचार किया जा रहा है।
आपात स्थिति में परिवारों के सामने सबसे बड़ी चुनौती अक्सर यह होती है कि गंभीर मरीज को किस अस्पताल या विशेषज्ञ विभाग तक पहुंचाया जाए और इस प्रक्रिया में कितना समय लगेगा।
प्रस्तावित नेटवर्क इसी अनिश्चितता को कम करने की दिशा में काम करेगा। अधिकारियों के अनुसार, पूरी व्यवस्था का मूल मंत्र “सही मरीज, सही विभाग और सही समय” होगा।
योजना में घटनास्थल पर शुरुआती मदद उपलब्ध कराने के लिए तीन-स्तरीय फर्स्ट रिस्पॉन्डर नेटवर्क बनाने का भी प्रस्ताव है।
इसमें पुलिस, एंबुलेंस कर्मी, अग्निशमन सेवा, हाईवे पेट्रोल, होमगार्ड और प्रशिक्षित नागरिकों को जोड़ा जाएगा। इसका उद्देश्य विशेषज्ञ चिकित्सा दल के पहुंचने से पहले ही जरूरी प्राथमिक सहायता उपलब्ध कराना है।
UPTEN को लंबे समय तक संचालित करने के लिए सरकार अलग बजट और मानव संसाधन नीति पर भी विचार कर रही है।
इसके तहत इमरजेंसी मेडिसिन ऑफिसर (EMO) कैडर बनाने, AIIMS जैसी वेतन संरचना लागू करने और विशेषज्ञ डॉक्टरों को प्रोत्साहन देने जैसे प्रस्ताव शामिल हैं।
‘गोल्डन ऑवर’ की अवधारणा इसी चिकित्सकीय आवश्यकता पर आधारित है कि गंभीर चोट, हार्ट अटैक, स्ट्रोक या किसी अन्य जानलेवा स्थिति में इलाज जितनी जल्दी शुरू हो, मरीज के बचने और बेहतर रिकवरी की संभावना उतनी अधिक रहती है।
शुरुआती समय में होने वाली देरी से स्थिति बिगड़ सकती है और आगे की जटिलताओं का खतरा बढ़ सकता है। ऐसे में प्रस्तावित नेटवर्क का जोर केवल तेज परिवहन पर नहीं, बल्कि समय पर सही उपचार शुरू कराने पर रहेगा।
