Madhya Pradesh Youth Dies in Ambulance After Oxygen Allegedly Runs Out, Mandla Administration Orders Probe
Tragedy unfolded on a highway in Madhya Pradesh’s Mandla district when a 21-year-old patient allegedly lost his life inside a government ambulance after its oxygen supply was exhausted during transit.
The heartbreaking incident, in which the young man breathed his last in his father’s arms while being rushed to hospital, has sparked allegations of serious lapses in emergency medical services and prompted the district administration to launch an investigation.
Following public outrage over the incident, the Mandla administration constituted a four-member inquiry committee to determine whether negligence at any stage contributed to the patient’s death.
The panel has been tasked with examining every aspect of the episode and fixing responsibility if any procedural failure is established.
The deceased, Ajit Baraiya, belonged to Sunhera Mal village under the jurisdiction of Bamhani police station.
Family members said he had been battling chronic kidney disease for a long time and was undergoing scheduled dialysis sessions at the Mandla District Hospital as part of his ongoing treatment.
Relatives stated that Ajit’s health deteriorated sharply on July 27, forcing them to take him to the Community Health Centre in Bamhani.
According to the family, doctors administered oxygen support there and advised that he be shifted immediately to the district hospital for specialised medical care.
The family has alleged that despite repeatedly informing emergency services about the seriousness of Ajit’s condition, the government-run 108 ambulance reached the health centre nearly an hour after the call for assistance was made.
They claimed valuable time was lost even though the patient required continuous oxygen support during transportation.
According to the family’s account, the ambulance initially carried oxygen, but the cylinder became empty before the vehicle reached Mandla.
They alleged that Ajit desperately struggled to breathe throughout the remaining journey and eventually died in his father’s lap before medical help could be provided.
Speaking through tears, Ajit’s father recalled the painful moments leading to his son’s death. He said his son fought for every breath until the ambulance reached Mandla, but by then it was too late.
Expressing anguish, he maintained that timely oxygen support could have saved Ajit’s life.
He further claimed that he had repeatedly informed the ambulance service about the urgency of the situation and had even expressed willingness to arrange an oxygen cylinder himself from Bamhani if necessary.
The grieving family also alleged lapses in the patient’s treatment before the transfer.
They claimed that despite Ajit’s rapidly worsening condition, their request for rescheduling his dialysis session was not accepted, resulting in critical delays before doctors finally referred him to the district hospital.
Taking serious note of the allegations, Mandla Collector Rahul Namdev ordered a detailed inquiry into the circumstances surrounding the death.
Describing the incident as deeply unfortunate, he said a four-member committee comprising medical professionals and a revenue department official had been formed and directed to submit its report without delay.
The Collector stated that the investigation would assess whether all operational norms prescribed for Basic Life Support (BLS) and Advanced Life Support (ALS) ambulances were strictly followed.
He said any violation of established procedures or evidence of negligence would invite stringent action against those found responsible.
Officials associated with the inquiry said the committee would not limit its examination to the medical treatment provided to the patient.
It would also scrutinise the maintenance records, technical condition and operational preparedness of the 108 ambulance service, including whether mandatory inspection of oxygen cylinders and other life-saving equipment had been completed before the ambulance was dispatched for the emergency call.

मध्य प्रदेश के मंडला जिले से सामने आई एक दर्दनाक घटना ने सरकारी आपातकालीन स्वास्थ्य सेवाओं की कार्यप्रणाली पर गंभीर सवाल खड़े कर दिए हैं।
अस्पताल ले जाए जा रहे 21 वर्षीय युवक की कथित रूप से सरकारी एंबुलेंस में ऑक्सीजन समाप्त हो जाने के बाद मौत हो गई।
पिता की गोद में तड़पते हुए बेटे की अंतिम सांसों ने पूरे परिवार को झकझोर दिया, जबकि घटना के बाद लापरवाही के आरोपों ने प्रशासन को जांच के आदेश देने पर मजबूर कर दिया।
मामले ने तूल पकड़ने के बाद जिला प्रशासन ने पूरे घटनाक्रम की जांच के लिए चार सदस्यीय समिति गठित कर दी है।
समिति को यह पता लगाने की जिम्मेदारी सौंपी गई है कि कहीं किसी स्तर पर लापरवाही या निर्धारित प्रक्रियाओं का उल्लंघन तो नहीं हुआ, जिससे मरीज की जान चली गई।
मृतक अजीत बरैया बम्हनी थाना क्षेत्र के सुन्हेरा माल गांव का निवासी था। परिजनों के अनुसार वह लंबे समय से किडनी की गंभीर बीमारी से पीड़ित था और नियमित रूप से मंडला जिला अस्पताल में डायलिसिस करवा रहा था।
परिवार ने बताया कि 27 जुलाई को अजीत की तबीयत अचानक अधिक बिगड़ गई। इसके बाद उसे बम्हनी सामुदायिक स्वास्थ्य केंद्र ले जाया गया, जहां चिकित्सकों ने उसे ऑक्सीजन सहायता उपलब्ध कराई और बेहतर उपचार के लिए तत्काल मंडला जिला अस्पताल रेफर कर दिया।
परिजनों का आरोप है कि मरीज की गंभीर स्थिति और ऑक्सीजन की तत्काल आवश्यकता की जानकारी बार-बार देने के बावजूद 108 एंबुलेंस लगभग एक घंटे बाद स्वास्थ्य केंद्र पहुंची।
उनका कहना है कि इस देरी के कारण बहुमूल्य समय नष्ट हुआ और मरीज की हालत लगातार बिगड़ती चली गई।
परिवार का दावा है कि यात्रा की शुरुआत में एंबुलेंस में ऑक्सीजन उपलब्ध थी, लेकिन रास्ते में सिलेंडर खाली हो गया।
इसके बाद अजीत पूरी यात्रा के दौरान सांस लेने के लिए संघर्ष करता रहा और जिला अस्पताल पहुंचने से पहले ही पिता की गोद में उसने दम तोड़ दिया।
घटना को याद करते हुए अजीत के पिता भावुक हो उठे। उन्होंने कहा कि उनका बेटा रास्ते भर हर सांस के लिए जूझता रहा।
उनका आरोप है कि यदि समय पर पर्याप्त ऑक्सीजन मिल जाती तो आज उनका बेटा जीवित होता।
उन्होंने यह भी कहा कि एंबुलेंस सेवा को लगातार मरीज की गंभीर स्थिति से अवगत कराया गया था और जरूरत पड़ने पर वह स्वयं बम्हनी से ऑक्सीजन सिलेंडर की व्यवस्था करने को भी तैयार थे।
परिवार ने उपचार व्यवस्था पर भी सवाल उठाए हैं। उनका कहना है कि अजीत की हालत लगातार बिगड़ने के बावजूद डायलिसिस की तिथि बदलने के उनके अनुरोध पर ध्यान नहीं दिया गया।
उनका आरोप है कि अस्पताल रेफर करने में हुई देरी ने भी मरीज की स्थिति को और गंभीर बना दिया।
घटना की गंभीरता को देखते हुए मंडला कलेक्टर राहुल नामदेव ने विस्तृत जांच के निर्देश दिए।
उन्होंने इस घटना को अत्यंत दुर्भाग्यपूर्ण बताते हुए कहा कि चिकित्सकों और राजस्व विभाग के एक अधिकारी को शामिल कर चार सदस्यीय समिति गठित की गई है, जिसे जल्द से जल्द अपनी रिपोर्ट प्रस्तुत करने के लिए कहा गया है।
कलेक्टर ने स्पष्ट किया कि जांच के दौरान यह भी परखा जाएगा कि बेसिक लाइफ सपोर्ट (बीएलएस) और एडवांस्ड लाइफ सपोर्ट (एएलएस) एंबुलेंसों के संचालन से संबंधित सभी सरकारी मानकों और निर्धारित प्रोटोकॉल का पालन किया गया था या नहीं।
यदि किसी भी स्तर पर लापरवाही या नियमों के उल्लंघन की पुष्टि होती है तो संबंधित अधिकारियों और कर्मचारियों के विरुद्ध कड़ी कार्रवाई की जाएगी।
जांच से जुड़े अधिकारियों ने बताया कि समिति केवल मरीज को दिए गए उपचार की समीक्षा तक सीमित नहीं रहेगी।
एंबुलेंस की तकनीकी स्थिति, रखरखाव व्यवस्था, संचालन प्रक्रिया तथा आपातकालीन सेवा शुरू करने से पहले ऑक्सीजन सिलेंडर सहित सभी जीवनरक्षक उपकरणों की अनिवार्य जांच की गई थी या नहीं, इन सभी पहलुओं की भी विस्तार से पड़ताल की जाएगी।
