SRN Hospital Prayagraj Surprise Inspection: Commissioner Gives 4 Days to Fix Lapses
Patients allege being sent outside for medicines and tests; poor sanitation, weak infection control, and unattended patients also raise concerns.

PRAYAGRAJ: The administration has given Swaroop Rani Nehru Hospital (SRN) four days to overhaul deficiencies in patient care and hospital management after a surprise inspection exposed shortcomings across several critical areas, including sanitation, infection control, equipment maintenance and emergency response.
Divisional Commissioner Dr Lokesh M. carried out the inspection on Tuesday along with Additional Director Health Dr Rakesh Sharma, Prayagraj Chief Medical Officer Dr Arun Tiwari, SRN Hospital CMS Dr Neelam and other officials.
Rather than limiting the exercise to administrative areas, the team visited wards, OPDs, diagnostic facilities and the trauma centre and sought feedback directly from patients and attendants.
One of the most significant issues emerged from conversations with patients in the orthopaedic wards. Several patients said they had been given prescriptions for medicines to be purchased outside the hospital.
Some also produced documents relating to X-rays and other investigations conducted externally.
Officials questioned the hospital administration about the complaints, while doctors attributed the shortage of some essential medicines to inadequate supplies from the corporation.
The inspection also brought patient-safety concerns to the fore.
In the trauma centre, emergency-care arrangements were found to be slower than expected, while cleanliness was below the required level.
Cleaning work was reportedly underway when the commissioner arrived.
The administration was directed to ensure that emergency patients receive prompt attention rather than having to wait for basic assistance.
At the trauma triage facility, medicines, injections, and medical supplies were not being maintained according to the prescribed standard operating procedure.
Dust and unclean surfaces were noticed, and biomedical waste was not properly separated. Forceps and other instruments had not been cleaned adequately, while the liquid used for cleaning them was found to be old.
Antiseptic arrangements were likewise considered deficient.
The condition of some patient beds added to the concern.
Bedsheets bearing dirt and blood stains were noticed, creating a potential infection-control risk.
Dr Lokesh M. told resident and intern doctors that the hospital caters to a substantial number of poor and financially vulnerable patients and that maintaining a hygienic and safe treatment environment must remain a priority.
A separate lapse was noticed in the minor operation theatre and dressing room near the trauma centre, where a woman with an open wound was found without adequate attention.
The concerned staff were instructed to ensure continuous and appropriate care of patients undergoing treatment.
An elderly critically ill patient receiving oxygen was also found waiting near the X-ray section without proper assistance.
The patient’s attendant did not have the required X-ray slip, which had delayed the process.
The commissioner ordered staff to arrange the examination immediately and ensure that the patient was taken to the appropriate ward afterwards.
The X-ray room itself came under criticism for poor housekeeping.
Dust and waste were found around the machine, while a broken chair, scrap-filled cartons, helmets, and other unnecessary items occupied the area.
Old prescriptions had also been left in a pile.
The technician concerned was reprimanded and ordered to clear the room and maintain it in accordance with the prescribed SOP.
In another area of concern, patients were reportedly being handed injection doses in advance for 24 hours.
The commissioner objected to the practice and instructed staff to follow established procedures governing the storage, handling and administration of medicines and injections.
The ENT OPD also failed to meet expected standards. Examination instruments and forceps were not found to be adequately sterilised, while bedsheets and materials such as cotton and gauze were lying in an unclean and disorganised condition.
Staff were reminded that sterilisation and infection-control measures cannot be compromised in patient-care areas.
Complaints were also raised by some patients and attendants in the women’s orthopaedic ward, who alleged that employees had sought money for certain minor services.
These allegations, along with prescriptions and reports shown by patients, were brought to the commissioner’s attention.
Dr Lokesh M. has instructed CMS Dr Neelam to bring hospital operations in line with the prescribed protocols and SOPs within four days.
A detailed investigation has been assigned to Additional Director Health Dr Rakesh Sharma.
If the inquiry establishes responsibility against any officer or employee, the report will be forwarded to the government for action according to the rules.
The commissioner has also ordered another surprise inspection after four days to check whether the shortcomings have actually been rectified.

प्रयागराज: स्वरूपरानी नेहरू चिकित्सालय (एसआरएन) में मरीजों को बेहतर और सुरक्षित इलाज उपलब्ध कराने की व्यवस्था पर सवाल खड़े करने वाली कई कमियां औचक निरीक्षण में सामने आई हैं।
दवाओं और जांच से जुड़ी शिकायतों के अलावा सफाई, संक्रमण नियंत्रण, उपकरणों की देखभाल और आपातकालीन मरीजों की त्वरित सहायता में भी खामियां मिलीं।
मंडलायुक्त डॉ. लोकेश एम. ने अस्पताल प्रशासन को चार दिन के भीतर व्यवस्थाएं सुधारने का निर्देश दिया है।
निरीक्षण के दौरान सबसे अहम जानकारी मरीजों और उनके तीमारदारों से बातचीत में सामने आई।
मेल और महिला ऑर्थो वार्ड में कई मरीजों ने बताया कि उन्हें दवाएं बाहर से खरीदने के लिए पर्चे दिए गए।
कुछ मरीजों ने बाहर से कराई गई एक्स-रे और दूसरी जांचों से संबंधित पर्चियां व रिपोर्ट भी अधिकारियों को दिखाईं। इस पर मंडलायुक्त ने अस्पताल प्रशासन से जवाब मांगा।
चिकित्सकों ने आवश्यक दवाओं की पर्याप्त आपूर्ति नहीं होने को इसका एक कारण बताया।
ट्रॉमा सेंटर में भी व्यवस्थाओं को लेकर गंभीर सवाल सामने आए।वहाँ सफाई का स्तर संतोषजनक नहीं था और निरीक्षण के समय ही सफाई का काम कराया जा रहा था।
आपातकालीन मरीजों की देखभाल और तत्काल प्रतिक्रिया की व्यवस्था भी अपेक्षित गति से काम करती नहीं मिली।
मंडलायुक्त ने स्पष्ट किया कि गंभीर मरीजों को इलाज और सहायता के लिए अनावश्यक इंतजार नहीं करना पड़ना चाहिए।
ट्रॉमा ट्रायेज रूम में मेडिकल सामग्री के रख-रखाव ने भी अधिकारियों को निराश किया।
दवाओं और इंजेक्शनों के साथ उपकरणों को निर्धारित एसओपी के अनुसार रखने की व्यवस्था नहीं थी। धूल और गंदगी मिलीं, जबकि बायोमेडिकल कचरे का पृथक्करण भी ठीक ढंग से नहीं किया गया था।
फोर्सेप्स सहित कुछ उपकरणों की सफाई में कमी मिली और उनकी सफाई में इस्तेमाल होने वाला लिक्विड पुराना पाया गया। एंटीसेप्टिक व्यवस्था भी निर्धारित मानकों के अनुरूप नहीं थी।
बेडशीट की स्थिति ने संक्रमण नियंत्रण को लेकर चिंता बढ़ा दी। कुछ चादरों पर गंदगी और खून के निशान मिले।
मंडलायुक्त ने रेजिडेंट और इंटर्न डॉक्टरों को मरीजों के लिए स्वच्छ वातावरण सुनिश्चित करने की जिम्मेदारी याद दिलाई।
उन्होंने कहा कि अस्पताल में आर्थिक रूप से कमजोर वर्ग के बड़ी संख्या में मरीज आते हैं और उनके उपचार में सुरक्षा व स्वच्छता से कोई समझौता नहीं होना चाहिए।
ट्रॉमा सेंटर के पास माइनर ओटी और ड्रेसिंग रूम में भी स्थिति ठीक नहीं मिली। खुले घाव का उपचार करा रही एक महिला मरीज पर्याप्त देखभाल के बिना मिली।
संबंधित चिकित्सा कर्मियों को तत्काल मरीजों की निगरानी और उपचार में लापरवाही नहीं बरतने के निर्देश दिए गए।
एक अन्य मामले में एक्स-रे विभाग की ओर जाते समय मंडलायुक्त की नजर ऑक्सीजन पर चल रहे एक बुजुर्ग गंभीर मरीज पर पड़ी, जो सहायता के इंतजार में था।
पूछताछ में पता चला कि मरीज के अटेंडेंट के पास एक्स-रे की पर्ची नहीं थी।
मंडलायुक्त ने तत्काल जांच कराने और मरीज को बाद में संबंधित वार्ड तक पहुंचाने की व्यवस्था करने को कहा।
एक्स-रे कक्ष में भी अव्यवस्था सामने आई। मशीन के आसपास धूल और गंदगी के साथ टूटी कुर्सी, स्क्रैप से भरे कार्टन, हेलमेट और अन्य गैरजरूरी सामान रखा था।
पुरानी पर्चियां भी एक स्थान पर पड़ी थीं। संबंधित टेक्नीशियन को फटकार लगाते हुए कमरे की तत्काल सफाई और अनावश्यक सामान हटाने के निर्देश दिए गए।
मरीजों को दवाएं और इंजेक्शन देने की प्रक्रिया पर भी सवाल उठे।
निरीक्षण में यह जानकारी मिली कि 24 घंटे की इंजेक्शन डोज मरीजों को पहले ही उपलब्ध करा दी जाती है।
मंडलायुक्त ने इस व्यवस्था पर नाराजगी जताते हुए दवाओं और इंजेक्शनों के रख-रखाव व इस्तेमाल में निर्धारित एसओपी का पालन करने को कहा।
ईएनटी ओपीडी में जांच के लिए इस्तेमाल होने वाले उपकरणों और फोर्सेप्स की स्टेरिलाइजेशन व्यवस्था पर्याप्त नहीं मिली।
बेडशीट गंदी थी और कॉटन व गॉज जैसी सामग्री भी साफ-सुथरे एवं व्यवस्थित तरीके से नहीं रखी गई थी। स्टाफ को संक्रमण नियंत्रण के मानकों का सख्ती से पालन करने की हिदायत दी गई।
महिला ऑर्थो वार्ड में बातचीत के दौरान कुछ मरीजों और परिजनों ने छोटी सेवाओं के बदले कर्मचारियों द्वारा पैसे मांगे जाने की शिकायत भी की।
मरीजों की ओर से प्रस्तुत दस्तावेजों और अन्य शिकायतों को मंडलायुक्त ने संज्ञान में लिया।
मंडलायुक्त डॉ. लोकेश एम. ने एसआरएन की सीएमएस डॉ. नीलम को चार दिन के भीतर सभी कमियों को दूर कर अस्पताल की व्यवस्थाएं निर्धारित प्रोटोकॉल और एसओपी के अनुरूप करने को कहा है।
अपर निदेशक स्वास्थ्य डॉ. राकेश शर्मा को मामले की विस्तृत जांच कर रिपोर्ट देने का निर्देश दिया गया है।
जांच में किसी अधिकारी या कर्मचारी की जिम्मेदारी तय होने पर नियमानुसार कार्रवाई के लिए रिपोर्ट शासन को भेजी जाएगी।
सुधार की वास्तविक स्थिति देखने के लिए चार दिन बाद अस्पताल का दोबारा औचक निरीक्षण भी किया जाएगा।
