Speaking to American radio host Glenn Beck on Wednesday, Trump said he assessed that the Iranian leader had survived.
According to the US president, Khamenei was badly injured, particularly on the left side of his body.
Trump referred to serious injuries involving his arm and leg but said he did not believe the Iranian leader had died.
Trump added that if Tehran were concealing Khamenei’s death, Iranian authorities had managed the deception remarkably effectively.
The uncertainty dates back to the February 28 US-Israeli attacks, which, according to the supplied information, killed Khamenei’s father, Ayatollah Ali Khamenei, and other senior Iranian figures.
Mojtaba subsequently emerged as Iran’s Supreme Leader, but his disappearance from public life soon became a major source of speculation.
Earlier reports suggested that he had been critically wounded and taken to a secure location.
Later claims that he had been hospitalised and was close to death further intensified questions about his survival.
His absence from his father’s funeral in July added to the uncertainty. While three of Ayatollah Ali Khamenei’s sons — Masoud, Meysam and Mostafa — were publicly seen during the mourning ceremonies, Mojtaba did not appear.
Iranian authorities have continued to present him as the country’s supreme leader, but the information available about his activities has been limited.
A recent video circulated by Iran’s Fars news agency was presented as rare evidence that Khamenei remains active.
The undated footage reportedly showed him observing a student’s academic presentation and commenting on the work.
However, the recording’s date and authenticity have not been independently established.
Trump used the same interview to project confidence in Washington’s broader approach towards Iran.
He said US military operations and economic pressure were moving towards what he expected would be a major success.
He also sought to downplay the scale of disruption in the Strait of Hormuz, claiming that shipping was continuing through the strategic waterway.
The status of the passage remains a major international concern because Iran’s military activity has significantly affected commercial navigation in the region.
Trump said the waterway was functioning and claimed that threats from mines had been removed.
Meanwhile, diplomatic discussions involving Iran and Oman are reported to be continuing over arrangements for commercial vessels.
Senior representatives of the two countries have been engaged in efforts to establish a framework for managing ship movement through the critical maritime corridor.
For now, however, one of the most important questions remains unresolved: the man officially leading Iran has not been seen publicly for months, and there is still no independently verified account of his location or medical condition.
Trump’s statement has offered one answer, but without a confirmed appearance by Mojtaba Khamenei, speculation over the Iranian leader’s fate is likely to continue.
